- Fill out the Crew Registration Packet (you'll need a member's only
password). More information is available below.
- Fill out the Athletic Packet (which includes taking a Sports Physical).

2005-2006 Crew Registration
Packet:
The entire registration packet is available in the Members
Only section. You can ask any of the members
of the current crew team for the username/password (folks
on the Contacts page are all good candidates).

Forms needed to be able to
go out on a boat : Both you and your parent/guardian have to sign these forms before you can participate in the JLAC practices. These forms are included in either the Crew Registration Packet or the BHS Athletic Packet.
BHS Crew Release from Liability.
JLAC Waiver form.
Swimming Proficiency Form.
Interscholastic Athletic
Team Emergency Card. <-
bring THREE copies.
ATHLETIC PACKETS & SPORTS PHYSICALS:
Sports physicals can be obtained from your private physician or at the
Berkeley High Health Center--located near the Bancroft gate. There is
no charge for the services at the Berkeley High Health Center --call 644-6965
for an appointment.
You must have the entire Athletic Packet filled out (including having
your doctor or the health center fill out the physical examination form)
before you can race with the team. Athletic Packets are available in the
Health Center and in the Administration Office.
All the Crew forms in the Registration Packet are available in the Member's
Only section. If you are a new novice, just ask any current Berkeley
Crew member for the username and password (all the folks on the Contacts
page are good candidates).
FORMS - Adobe Acrobat (.pdf) format:
All forms are in Adobe Acrobat (.pdf) format - which is a cross-platform way of getting fancy text to everyone. You can save/print these Adobe Acrobat files using the icons that appear just above the form itself (after you've double-clicked/opened the form) - You don't want to use your "regular menu items" - i.e.those across the top of your browser.If you need a (free) copy of the Acrobat Reader, it's available here. |
