Forms needed to be able to go out on a boat

Both you and your parent/guardian have to sign these forms before you can participate in the JLAC practices. These forms are included in either the Crew Registration Packet or the BHS Athletic Packet.

BHS Crew Release from Liability.

JLAC Waiver form.

Swimming Proficiency Form.

Interscholastic Athletic Team Emergency Card.      <- bring THREE copies.
