Carpool Driver Notification and Consent Forms

Our school district requires parents or guardians to submit three forms to allow students to travel in private vehicles to races and other official school events. On these forms, drivers must:

• Provide vehicle, license and insurance information for each vehicle to be used
• Acknowledge the requirements and responsibilities involved in driving students in a private vehicle - each driver must complete this
• Grant consent for minor children to ride in private cars, releasing the school district from liability – please complete a separate form for each child.

Please print these forms, sign them, and return them to your son's or daughter's coach prior to the start of race season. Remember to include photocopies of your driver's license and vehicle insurance card.

Berkeley High Crew cannot allow anyone without these signed forms to take part in a regatta. NO EXCEPTIONS can or will be made.  These are similar to the slips you sign to allow your child to go on a field trip. The same rule applies   - no slip, no trip.
